This online form is to update your address, if necessary, and request a ballot be
mailed to you or
picked up at either location listed below:

If you are picking up a ballot for another person, they must sign a note explicitly giving you permission to do so or complete this optional form authorizing you to pick up the ballot. You must bring a copy of the signed note or form with you to pick up a ballot on their behalf.This replacement ballot order form is for Multnomah County registered voters. If your residence is in another county, please contact that
county Elections office.
For Will Call orders, during
business hours, a ballot will usually be available within two hours. Orders placed outside of business hours or less than two hours before the end of the business day will usually be available by 10am the next business day.
Please wait for a confirmation email that your ballot is ready for pickup before arriving.
If there are any problems with your request, the Elections office will contact you at the phone number or email address you provide.
